Hampton Public School is offering on-going registration for students joining our 2023/2024 Preschool  and Kindergarten​ programs.​     

In order for the registration process to be complete, and compliant​ ,​ please note the eligibility requirements below, along with a list of forms needed.  ​  

Eligibility 

  • Kindergarten registrants must be 5 years of age ​prior  to October 1, 2023;​     

  • Preschool registrants must be 3​ or 4 years of age and not older than 5 years of age prior to Oct. 1, 2023; 

  • Child must be a resident of Hampton Borough; 

  • Child must be potty trained  (for Kindergarten)

Required Forms*  

  • Original birth certificate with seal* 

  • Up-to-date Immunization records, including Hepatitis B and flu vaccines  

  • Universal Child Health Form - print form and bring to your physician

  • Elementary Health History - print form

  • Proof of Residency, i.e. copy of lease or mortgage statement 

  • Student Enrollment Registration Form  

Please scan and return all forms via email to Ms. Gizis in the Main Office dgizis@hamptonpublicschool.org​.​ *Copies​ will be accepted, however, the original form must be presented prior to the first day of attendance (by appointment only).
